Second surgery stage is stenting or through a skin transluminal angioplastic with stenting.  And surgery stage as entering in vessel balloon with catheter and to stenosis place, bloating balloon and widening,  and a putting before a bloating a filter umbrella ,taking  are  chopping  plaques,as against entering   are pieces in distally having brain vessels and theirs blockage , as for a future a stent putting and removing catheter with balloon and umbrella, as narcosis here is local.

And Surgeries are not having consent a which surgery is effectivity. Endarterectomies  is more hard and injury, second is easy, and less injury, but dates are showing,that after stenting are happening  a restenosis as repeating stenting of surgeries vessel through a few months or years.
